Before the First Appointment

 

The first step is contacting our office to schedule your child's initial evaluation. Our Scheduling Coordinator will confirm the date and time of the appointment, as well as the name of the therapist who will be meeting with you and your child.

 

Next, we ask that you complete the Intake Form at least one week before your scheduled appointment. This allows the therapist to review the information and for us to identify any additional supports or resources we may provide when you come in. If you have any medical records, reports or additional information related to your child's development, you may attach it to the Intake Form or bring in hard copies when you come in for the evaluation.

 

We also ask that you download our Notice of Privacy Practices, which provides you with information about your rights and our responsibilities to keep your family's health information confidential, and to complete the Consent and Release of Information Form. This allows us to share information about your child's services with their pediatrician, school staff, and anyone else you name.

 

 

 

On the Day of the First Appointment

 

Please arrive 10-15 minutes early if possible, to ensure that your child has enough time to transition into the treatment room if needed. If you happen to run late, please contact us immediately so that we may reschedule the session if needed. If you are more than 15 minutes late and we do not hear from you, we will automatically cancel the appointment.

 

During the appointment, your therapist will review your child's medical history and other information you provided when you completed the Intake Form. Your therapist will also observe your child and ask you any additional questions that will enable them to develop a plan that best supports your entire family. Once your therapist has completed the evaluation, they will give you information about your child's plan of care. This plan will include information you provided about any areas of development you have concerns about or any daily routines your child finds difficult or challenging. 

 

If your therapist recommends services, the next step is scheduling ongoing weekly appointments based on their recommendations and your availability. Before your first appointment is over, one of our staff members will confirm when weekly appointments may begin, or you may let us know when you would like to begin weekly appointments once we have confirmed a date and time that works best for you.
