Getting Started

If interested in setting up an appointment, We ask that you first fill out an Intake Form. Once we receive your information our Care Specialist will call you within a weeks time to confirm the date and time of the next available appointment.


If you have any medical records, reports or additional information related to your child's development, you may attach it to the Intake Form or bring in hard copies when you come in for the evaluation.


We also ask that you download our Notice of Privacy Practices, which provides you with information about your rights and our responsibilities to keep your family's health information confidential and to complete the Consent and Release of Information Form. This allows us to share information about your child's services with their pediatrician, school staff, and anyone else you name.




Your first appointment will be an hour long evaluation. Please arrive 10-15 minutes early if possible, to ensure that your child has enough time to transition into the treatment room if needed. If you happen to run late, please contact us immediately so that we may reschedule the session if needed. If you are more than 15 minutes late and we do not hear from you, we will automatically cancel the appointment.


Our comprehensive evaluations are set up to look and feel like play. The evaluator will use the information you have provided in the intake form to gather toys that will make your child feel comfortable. While we play with and observe your child we will ask you questions related to your concerns regarding your child’s development. These questions will enable them to develop a plan that best supports your entire family. Click here to review a list of areas that your therapist will assess during your child's evaluation. By the end of the evaluation, we will provide you with our recommendation for further treatment as well as additional information to resources in the community


If your therapist recommends services, the next step is scheduling ongoing weekly appointments based on their recommendations and your availability. Before your first appointment is over, one of our staff members will confirm when weekly appointments may begin, or you may let us know when you would like to begin weekly appointments once we have confirmed a date and time that works best for you.
